It's cool, but it's not very realistic (technical inaccuracies, oversimplifications, lack of physical behaviours of certain things, rudimentary driving, tycoon/management game mechanics (a lot of old cars are reworked/renovated and sold at a loss — ask my IRL car mechanic friend), etc.). Still, relatively useful for IRL car mechanic noobs.
It's moddable, which is awesome. Still, there are weird limitations, e.g. a Fiat 126p mod (very old car, the last one was produced in 1991) has an OBD port (which is nonsense). I very much doubt that this is done by the modder non-deliberately, which means that there are some modding limitations that necessitate introducing nonsensical stuff, which isn't cool for games with the word "simulator" in their title.
Another thing that isn't cool is that there are some cars that are NOT reintroduced from older versions of the "game" (e.g. CMS2015). A significant example for me is the Fiat 126 (I wanted to own a 126p when I was in highschool 17 years ago). The weirder thing is that it isn't even licensed (named "Youngtimer"):
Thus, if you want to play with a vanilla 126, you need CMS2015, which is sad.
I recently purchased CMS2021 with almost all the DLCs (still haven't tried it), but I'll probably skip on upcoming versions.
While not as complex and offering much less freedom, it still has the Kerbal Space Program vibe I've been chasing for years.
P.S.
The intro cinematic (more precisely, the movement of the humans) reminded me the game cinematic technology of Diablo 2 (2000).

Was surprisingly fun and relaxing, even if you don't know much about cars. The game strikes a nice balance between realism and accessibility. It's definitely simplified to keep things fun so you're not drowning in overly complicated mechanics. It's realistic enough to teach you the basics, but not so much that you need to be a real-life mechanic to enjoy it.
10/10, would lose sleep again to rebuild an engine that purrs like a kitten.

Made the jump to CMS 2021 after 660 hours on the 2018, definitely recommend.

CMS is a simulator you HAVE to learn if you don't know anything about cars. After all, I don't expect a lot of people to understand what or where (for example) a camshaft is and where it is located. I can happily say that I'm lucky I grew up in a garage, so this game happens to be pretty enjoyable to me. However, if you know absolutely zero about car mechanics, I'd stay away from this simulator. It's PROBABLY not for you. Unless you are willing to learn. While the game may not be 100% accurate to real life, this game is laughably serious and sometimes tedious- If you screw up too badly early on and order the wrong parts for your repair (on the game), you may just run out of money. I've spent over an hour on one car trying to diagnose the problem. I put an entire engine back together just to miss one small piece, only to have to take it BACK APART again. Yes, it's that serious. It's like a puzzle. It requires a lot of focus and dedication which most people may find boring. However, I really enjoy finding and diagnosing problems in general; because it gives me some sense of enjoyment knowing that I am capable to do so. If you don't have the patience, don't try. Chances are, you'll become extremely frustrated at some point. If you enjoy, I can 100 percent recommend this game to you. Over time, it becomes even more rewarding when you make up enough money from jobs to go to the barns and purchase your own cars to repair and mod. I know this is the 2018 version and there is a newer version of CMS available which I will purchase next, but so far 18 hours in, I'm still loving it!
